Application Process

documentsigning If you are interested in becoming a Capital service provider, the first step is to submit your company information to Capital to see if you qualify

If you are selected to become a Capital Service Provider, you will be required to fill out/submit the following information:

  • An independent contractor agreement (form will be provided by Capital)

  • Proof of the following minimum insurance coverages:
    • Automobile insurance of at least $1mm
    • Umbrella/excess liability insurance of at least $1 million per occurrence
    • Workers Compensation insurance of at least $1mm per occurrence
    • General Liability Insurance of at least $1 million per occurrence and $2mm in the aggregate, with the following minimum sub-limits:
      • $100,000 for damage to rented premises
      • $5,000 medical expenses per person
      • $1 million personal and adv injury
      • $2 million products – comp/og agg
    • While all of this sounds complicated, it is all actually fairly standard and simple for your insurance agent or broker to handle. Capital would be happy to communicate directly with your insurance agent or broker to arrange.

  • A signed W-9 form (form will be provided by Capital)

  • A business questionnaire (form will be provided by Capital)

  • Your agreement to comply with Capital’s Code of Conduct (will be provided by Capital)
